Mental Health Intake Services

Services are available to individuals who live, work, or own property in Shawnee County regardless of ability to pay. Intake Services accommodate physical disabilities, and those in need of language and sign language interpreters.

~Single point of entry for all Valeo Services

~Walk-In Clinic M-F 8:00 am-3:00 pm, located at 400 SW Oakley

~Documentation and  Collection of pertinent information for Mental Health Assessment

~Referrals for Therapy, Medication, Case Management and other service appointments

~Photo ID & Proof of Household Income Required

~Medicaid, Medicare, Insurance, and Fee for Service accepted.

Philosophy

Intake Services receives all requests for services and information with the goal of assisting the individual in obtaining needed mental health services in a timely manner. The client is an informed participant who actively takes part in and agrees to the final treatment recommendations. It is the objective of Intake Services to determine if Valeo Behavioral Health Care is the appropriate provider; if not, to refer the client to appropriate community resources.

Overview of Intake Services

An Intake is the starting point to receive outpatient behavioral health services at Valeo. During an Intake, a clinician will complete an initial mental health assessment to determine appropriate mental health services, and will make referrals after the completion of the assessment. Referrals may be made to individual and/or group therapy and medication evaluation. For those clients meeting eligibility requirements, referrals can also be made to case management services, psychosocial groups and employment and education services.

Population Served

  • Valeo BHC serves adults age 18 and over.
  • Valeo BHC serves residents of Shawnee County or who report household income from within Shawnee County.
  • Valeo BHC offers accommodations for physical disabilities, sign language, and language interpreters as needed.

Preparing for an Intake, please bring:

  • Insurance card (co-pay expected at time of visit)
  • If no insurance, a minimum of $15.00 is expected at time of service
  • Proof of income
  • Medication List
