SOAR Team Case Manager


The SOAR I (SSI/SSDI Outreach, Access and Recovery) case manager will assist eligible clients in the SSI/SSDI application process using the SOAR model. Tasks may include interviewing client; coordinating application efforts with case managers, Social Security Administration, Disability Determination Services, and other involved parties; providing SOAR training and education to the community, and other tasks as requested.  Some availability after working hours may be necessary. 

Job Requirements:

This position requires: 1) a person with a Bachelor’s degree in social/human service field, 2) a person with an Associate of Arts degree in social/human service field with experience, or 3) an individual equally qualified by work experience or a combination of work experience and education, with one year of experience substituting for one year of education.  Additionally, this person must have excellent written and oral communication skills and be highly organized. Experience with persons with severe and persistent mental illness and/or chemical dependency is preferred.  This position requires an extensive knowledge of SSI / SSDI, insurance, and other public entitlement programs.  A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required.  

Salary Range: $18.92 – $21.50
