LINK Grant – Community Support Navigator


The primary function of the Community Support Navigator is to help individuals achieve short-term, goals based on individual wants and needs in the areas of daily living, financial, vocational/education, social supports, health, and leisure/recreational supports.  As part of the LINK Partnership, the Community Support Navigator will provide short-term case management services to persons enrolled in HealthAccess who have been identified through Stormont Vail Health’s analytics as needing assistance with social determinants of health to prevent rising risk.  Individuals served in this project will primarily be individuals who have received care within Stormont Vail Health (i.e., the Emergency Department, inpatient hospitalization or outpatient services).  This position is directly responsible to and supervised by the Team Leader of Case Management Services.

Job Requirements

Community Support Navigator services are provided by at least a person with a BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.  A BA in social work is preferred.  Experience with accessing community resources is required.  In addition, the individual must be 18 years of age or older; be able to pass a KBI security check and DCF Child and Adult Abuse Registry check. Must have reliable transportation with auto insurance required. This position requires knowledge of basic computer skills. Must be able to complete required WSU online trainings and program trainings. This position requires excellent organizational skills; the ability to be responsible and work independently; the ability to maintain flexibility and dependability in work schedule; positive interpersonal skills; the ability to communicate effectively both verbally and in writing.  Bi-lingual is preferred.  Must be able to perform day-to-day physical requirements of the position, including but not limited to- 20% of scheduled time; data entry, answering the phone and computer work, 80% of scheduled time; ability to lift up to 25 pounds, frequently moving around the building and community, repetitive moving, carrying and lifting as the job demands.   

Salary Range

$20.91 – $23.10 – $26.44


Mon-Fri – 8:00 am to 5:00 pm, but some flexibility is required.
