Responsibilities
The essential function of the Health Coordinator is to provide an assessment of need for medical care for Certified Community Behavioral Health Clinic (CCBHC) clients. The Health Coordinator will provide primary care screening and monitoring of key health indicators and health risk. The Health Coordinator will assist clients connect with needed services and supports and will monitor hospital and emergency department utilization to ensure necessary follow-up and coordination is provided.
Requirements
This position requires a Licensed Practical Nurse (LPN), Bachelor’s degree with healthcare experience, Registered Medical Assistant (RMA), or other health related certification (e.g., CMA, CNA, CPhT, etc.). Experience with physical health assessments and coordinating care preferred. Experience working with individuals with severe mental illness, substance use disorders, or co-occurring disorders preferred. Should be able to work independently in a fast paced environment, while also working collaboratively with team members and community partners. Desirable personal traits include dependability, excellent organizational skills, and the ability to communicate effectively both verbally and in writing. This position requires basic computer skills, the ability to sit for long periods of time, and the ability to lift at least twenty five pounds. Must be able to pass pre-employment background checks and KDADS Adult/Child Abuse Registries.

$18.29 – $21.20 – $24.68

$24.42 – $27.39 – $31.11
Hours
40 hours, some evenings hours may be required
