Responsibilities
The primary function of the CCM is to help persons achieve short-term, crisis related goals based on the persons wants and needs in the areas of daily living, financial, vocational/education, social supports, health, and leisure/recreational supports. As a member of the CCM team, the CCM will provide intensive short-term CM services to persons who are at risk of State hospitalization, individuals who need intensive wrap-around from exiting a hospital, and short-term CM to those who would likely become at risk without earlier intervention. This position is directly responsible to and supervised by the Team Leader of Crisis Diversion Services. Some availability in the evenings and weekends.
Job Requirements
Crisis Case Managers services are provided by at least 1.) a person with a BA/BS degree, 2.) a person with an Associate of Arts degree in Mental Health technology with experience, 3.) an LMHT with experience or 4.) one equivalently qualified by work experience substituting for one year of education. Experience with persons with severe and persistent mental illness and crisis case management preferred. In addition, the individual must be 18 years of age or older; be able to pass a criminal background check and KDADS Child and Adult Abuse Registry check. Must have reliable transportation with auto insurance required. This position requires knowledge of basic computer skills. Must be able to complete required KsTrain online trainings and program trainings. This position requires excellent organizational skills; the ability to be responsible and work independently; the ability to maintain flexibility and dependability in work schedule; positive interpersonal skills; the ability to communicate effectively both verbally and in writing. Must be able to perform day-to-day physical requirements of the position, including but not limited to- 20% of scheduled time; data entry, answering the phone and computer work, 80% of scheduled time; ability to lift up to 25 pounds, frequently moving around the building and community, repetitive moving, carrying and lifting as the job demands, as well as the ability to work on-call day, evening, night or weekend shifts based on the needs of the program.

$21.86 – $24.24 – $27.76

Tuesday – Saturday 10am – 6:30pm
